Health Form

Each camper, leader, staff member, and volunteer MUST have a completed health form on file. The health form requires a parent signature for those under the age of 19.

Important note: Summer 2009 campers should bring the completed health form with them to registration. You do not need to mail your health form to the camp offce. An attached copy of your medical insurance card is also mandatory.  Camper registration is not complete without a signed health form. If your camper is attending multiple sessions, please make a copy of the health form for each session that they are attending and bring a copy at each registration.
